- This form is used by persons travelling on behalf of C&C (Council and Committees) and claiming directly his/her travel expenses. Direct claims are unusual (see also Financial Rules 22.1 and 22.2)

- When having paid privately for something that the Association shall pay, this form is used

- When calling in sick, this form is used to keep track of the sickness period. This form is also used for Childcare absence
